NOTE: Unlike the previous version of the Appendices, these are NOT printer friendly. Check the Download Area for the individual items you need, or simply cut and paste from this page.

 

SRC Guild Application

Please fill out this application completely. Use lower case for all screen names to differentiate letters that look alike (i, l, 1, 0, o, etc.).  For items requiring several names, list the character name then the screen name, with the screen name in parentheses, and separate each pair of names from the others with a semicolon (;). This is to help ensure correct entry in the SRC database. Return the application as an email, not an attachment, and do not use colored text or background---black text on white background only.

Your application will be reviewed and a decision made as soon as possible (probably within 5 days), after which you will be contacted with your status.
